You will be asked for the following:

Ontario Health Card Number

A brief description of your problem

* If you are not comfortable providing some of this info over the phone, you can choose to provide it directly to the doctor at the time of the visit.

Step 2
You will receive a call within 1-2 hours of the housecall to confirm the visit.
Hours of Service:
Weekdays: 6-10 PM
Weekends: 9 AM- 11 PM
The exact time you will be seen will depend on the overall call volume and distribution on the day you call.
Step 3
Please have the following items ready for the doctor:

Valid Ontario Health Card
-
List of your current medications
Note: If you are called and there is no answer, we will try calling again. If there is still no answer, we will assume that you are not home or that you have made other arrangements, and the call will be cancelled.
*If you think it may be difficult for the doctor to access your home, it would be appreciated if someone could wait by the street in front of your home to escort the doctor in.
